Online Bullying Reporting Form
The Cincinnati Public School District is committed to providing a safe learning
environment for all students. Bullying incidents can be reported directly to a
trusted staff member at our school. This online form may also be used by CPS
students to report being bullied or by any adult or student who witnessed

All reports of harassment and violence are important and thoroughly investigated. The results are recorded in the PowerSchool logs. Those results will be reviewed if another complaint is received involving any of the same students.
Once submitted, CPS uses its best efforts to initiate an investigation within one
business day. Please keep in mind that this process can only
be as thorough as the information provided. The information that you provide on
this form will be sent to your child’s principal and to
the Customer Care Center.
If you fear that you or a student is in immediate danger, please report this incident to an
administrator or CPS school personnel as soon as possible.

Bullying is when someone repeatedly and on purpose says or does mean or hurtful things
to another person who has a hard time defending himself or herself (Olweus Bullying
Prevention Program).
